Facilities with inaccurate resident data or incomplete staffing data (497 facilities) were removed. Facilities with staffing levels in the lower 1 percent and the upper 2 percent (calculated separately for Medicaid-only facilities and Medicare-certified facilities) (1,211 facilities) were removed.
