12.1 Six Month Progress Report

In all Associateship Programs, an interim report on the progress of your research is required at the end of six months. At the appropriate time, you will receive a "Six-Month Progress Report," requesting a summary of your research program. Included in this report are the following questions:

12.1.1 -- Associateship Programs Functions

(1) Were the prestart materials and instructions satisfactory?

(2) Was your relocation processed in a satisfactory manner?

(3) If requested, was your stipend advance available when you
began tenure?

(4) Are you receiving your stipend regularly and on time?

(5) Are your travel requests and travel reimbursements processed promptly and satisfactorily?

(6) Are your questions to the Associateship Programs staff answered courteously and efficiently?

12.1.2 -- Laboratory Functions

(1) Was the laboratory ready to receive you and help you begin your tenure?

(2) Is your interaction with your Research Adviser and Laboratory Program Representative satisfactory?

(3) Is the space assigned to you adequate?

(4) Are you experiencing any problems in acquiring equipment, computer time, supplies, or technical support?

(5) Are you encouraged to plan for the publication of your research results in refereed journals?

(6) Are you able to participate in local or regional seminars, colloquia, or meetings?

(7) Are you encouraged to attend appropriate national or international scientific meetings?

(8) Have you encountered any influences at the laboratory that you find detrimental to your research?

12.2 Final Report

Shortly before the end of tenure, you will receive a copy of the Final Report format (as shown below) to complete and return to the Associateship Programs office not later than 15 days before the date on which your tenure ends. Your final stipend check will not be released until this report is received. Included in this report are the following questions and requests for information:

(1) Date

(2) Name (and identification number, if known)

(3) Name and location of laboratory or Center

(4) Dates of tenure

(5) Title of research project

(6) Research Adviser's name

(7) Are you on leave from a professional post?
If so, list position or title and address of facility.

(8) International posts held during tenure

(9) Programmatic travel during tenure

List each location and date.

(10) Scientific seminars, meetings, and/or consultations
List each location and date. List foreign meetings separately.

(11) Seminars or lectures delivered at universities and/or institutes
List each location and date.

(12) Meetings attended by specific invitation
List each location and date.

(13) Teaching, if any, as an Associate

(14) Work in progress

(15) Summary of research during tenure -- Give special attention to this item, which forms the basis of annual bliographic listing of Associates' work while on tenure. Limit the summary to 100 words or less, single spaced, so that it may be entered into a permanent data bank in the Associateship Programs office. Do not use Greek letters or mathematical signs and symbols.

(16) Publications and papers resulting from research as an Associate --

Provide a complete citation for each publication or paper, including author(s) name and initials, full name of journal, volume and page number, and year of publication.

(17) Patents applied for as a result of work as an Associate

(18) Future position and address or current forwarding address

(19) Appraisal of the Associateship Program -- Provide comments on your program and its usefulness to you and suggestions for improving the overall Associateship Programs.
