NOTE: Facilities with inaccurate resident data or incomplete staffing data (497 facilities) were removed. Facilities with staffing levels in the lower 1 percent and the upper 2 percent (1,211) (calculated separately for Medicaid only facilities and Medicare certified facilities) were removed.
