A-1 A P P E N D I X A Survey Questionnaire Email to Potential Participants From: NCHRP Staff To: Members of AASHTO Standing Committee on Construction Members Subject: NCHRP Project 20-05/Synthesis Topic 50-07: Electronic Ticketing of Materials for Construction Management Dear Members of the AASHTO Committee on Construction Members: The National Academy of Sciences (NAS) through the Transportation Research Board (TRB), conducts studies related to transportation to benefit state departments of transportation. Within TRB, the National Cooperative Highway Research Program (NCHRP) works with the American Association of State Highway and Transportation Officials (AASHTO) and the Federal Highway Administration to identify, fund, execute and publish these projects. The NCHRP Synthesis Program collects information on the current state of the practice for identified issues in transportation. I am writing to you now to request that you share information relative to your agency's experience regarding the use of electronic tickets (or e-ticketing), which is the subject of NCHRP Project 20-05/Synthesis Topic 50-07: Electronic Ticketing of Materials for Construction Management. This synthesis will identify state DOT use, evaluation, and perceptions of e-ticketing technology. A survey has been prepared as part of the information gathering on that topic. Therefore, we are sending this survey to you in hopes that you can forward it along to individuals in your agency who are in a position to answer the questions. Even if your state does not have experience with e-ticketing, your responses are still needed. If you believe a consultant is your state's subject matter expert on e-ticketing, please contact me by phone or email (contact info below) for further information. Please complete the survey at the following link, https://uky.az1.qualtrics.com/jfe/form/SV_5nbzN861j9BS6c5, by Friday, March 1st. The survey is estimated to take 30 minutes or less to complete. This synthesis is being conducted by Gabe Dadi and Roy Sturgill from the University of Kentucky and Kentucky Transportation Center. If you have any questions about the survey or would like to discuss the topic, please feel free to contact Dr. Gabe Dadi: Gabe Dadi, Ph.D., PE Assistant Professor University of Kentucky 151C Raymond Building Lexington, KY 40506-0281 gabe.dadi@uky.edu 859-257-5416 Thank you for your time and cooperation in support of this important project. Your cooperation in completing this survey will help ensure the success of the effort.

A-2 Electronic Ticketing of Materials for Construction Management transportation agencies. The purpose of this synthesis is to collect information related to e- ticketing usage, implementation, tools, knowledge gaps, and future needs to holistically evaluate the benefits of the technologies as well as best practices for states to consider in their potential adoption of the technology. For any questions/comments please contact: Gabriel B. Dadi Email: gabe.dadi@uky.edu Phone: (859) 257-5416 1. Please identify your contact information. Collecting paper load delivery tickets at highway construction projects is an outdated practice that exposes construction inspectors to many safety hazards. State transportation agencies (STAs) have looked for decades toward automating this process through electronic ticketing, or e-ticketing.

The TRB National Cooperative Highway Research Program's NCHRP Synthesis 545: Electronic Ticketing of Materials for Construction Management examines the current state of practice of e-ticketing among STAs.

Further, a survey was taken as part of the synthesis and found that 10 states have completed projects with e-ticketing and additional ones are piloting the technology.
