The questionnaires are distributed with the cooperation of the Deans of Graduate Schools and filled in by the graduates when they complete all requirements for their doctoral degrees. The doctorate recipients provide data about their birth date and place, sex, citizenship, marital status, racial or ethnic group, educational background from high school to doctorate, sources of financial support in graduate school, and postgraduation plans.

The Survey is conducted annually by the Commission on Human Resources of the National Research Council in cooperation with the American Council of Learned Societies and the Social Science Research Council. The form is distributed with the cooperation of the Graduate Deans and filled out by all graduates who have completed requirements for their doctoral degrees.
